RE: COVID-19 Vaccination clinic for Cohen students aged 12 and over

The City of Bangor Public Health is teaming with Penobscot Community Health Center to offer a COVID-19 vaccination clinic at  the William S. Cohen School for students aged 12 or older on Friday, May 21.

Clinic Dates: Friday, May 21 (1st dose)  and Friday, June 11 (2nd dose)

Location: William S. Cohen School Gymnasium

Time: 9:00-10:30

To arrange for your eligible Cohen student to participate in the clinic:

1. Review the attached Pfizer COVID-19 Vaccine Fact Sheet.

Youth Vaccination

1. Submit the appropriate Consent Form to the Cohen Main Office as soon as possible as the clinic takes place on May 21.
